Ayurveda, the ancient Indian medical science, presents a comprehensive and systemic approach to disease management through its dual methodology: Samshodhana (purificatory therapy) and Samshamana (pacificatory therapy). Within this spectrum, Panchkarma stands out as a key purification protocol that embodies the Samshodhana approach. This paper explores the interrelationship between these two therapeutic paradigms within the framework of Ayurvedic pathology. It delves into the philosophical and physiological rationale behind their application, analyzes the steps of Panchkarma in connection with the stages of disease progression (Shatkriyakala), and discusses their complementary roles in restoring doshic balance. The paper also presents contemporary perspectives on Panchkarma's relevance in chronic and lifestyle diseases, outlines prevailing challenges in its global practice, and highlights its potential in integrative health systems.
