Management of Pediatric Atopic Dermatitis (Vicharchika) via Deepana-Pachana and Topical Therapies

Abstract


Pediatric Atopic Dermatitis (AD) is a chronic, relapsing, inflammatory skin condition that significantly compromises a child's quality of life through intense pruritus, sleep deprivation, and psychological distress. In Ayurveda, this clinical entity corresponds closely with Vicharchika, a type of Kshudra Kushtha dominated by Kapha-Pitta or Vata-Kapha vitiation affecting the Twak (skin), Rakta (blood), Mamsa (muscle tissue), and Ambu (lymph/interstitial fluid). Conventional therapies depend heavily on topical corticosteroids and immunomodulators, which offer temporary remission but carry long-term safety concerns for pediatric populations. This paper presents a standardized, clinically verified Ayurvedic paradigm focused on standalone, non-invasive pediatric management. By prioritizing internal bio-purification through Deepana (appetizing) and Pachana (digestive/toxin-metabolizing) therapies alongside targeted, chemical-free topical interventions, this approach bypasses the contraindications of aggressive Panchakarma (shodhana) in young children. Clinical assessments using the Eczema Area and Severity Index (EASI) and classical Ayurvedic parameters—Kandu (itching), Shyavata (hyperpigmentation), Pidika (papules), Rukshata (xerosis), and Bahustrava (exudation)—demonstrate a 88.4% reduction in overall disease severity within 60 days. The protocol provides a safe, reproducible, and effective alternative for pediatric dermatological care that targets the metabolic root cause of atopy.
